Prerequisite Health and Safety for all sub-contractors
We hereby request that the following be in place before work commences on any site.
- Current letter of good-standing (from Workman’s Compensation or FEM)
- Risk assessments for tasks (site specific)
- Copies of appointments for the site
- An agreement with mandatory 37(2) form
- Toolbox talks (safety talks) to be carried out by yourselves for your employees on a weekly basis and these must be documented.
- A list of all your electrical equipment and tools. These must have registers/checklists for them.
- Inductions to be arranged (with CAM) for all employees on site as well as new employees.
- Medicals for persons operating plant and persons working at heights.
- Plant operators to also have in their possession a copy of their license and his certificate of competence.
- Employer’s reporting of incident form, for the purpose of an injured employee needing medical attention (hospital or ambulance).
- Copy of ID of all employees on site.
